Make Sure Your Student Health Records Are Up To Date

The MPCS nurses clinic is pleased to launch a new portal for student health-related information. Accessed through your FACTS portal, the Magnus system makes it simple to keep your child's important health documentation uploaded and up to date. Parents are asked to update student health information every year through the re-registration process. With Magnus, you can add more extensive details concerning care for your child, as well as add updates such as athletic physicals or allergy forms on the fly. 

Please note, If you do not fill out the Magnus Health information for your student, they may not be able to participate in ANY off-campus events or athletics. 

Parents MUST check the health information MPCS has on file for each student now and for the upcoming 2025-2026 school year. Simply go to your FACTS Portal and follow the instructions below.

How to Update Your Child's Medical Information

 

1. Login to your FACTS Portal through the Eagle Family Portal (on website) or at the button below.

2. Under the "Student" tab on the left side navigation, locate and tap "Medical." 

 

3. Select the "Login" button for Magnus Health portal. This is a secure login direct from FACTS and no additional password is required when accessing through FACTS. 

 

4. You will be redirected to the Magnus Health Portal to access your student's health information. Watch the introductory video to learn more about Magnus. 

 

5. Click to close. You'll then see the clinic "front desk" which shows your child(ren)'s names and links to their individual health records to complete an update. Some information must be updated every year.  

 

6. Choose a student and click "Complete Update" to begin reviewing information on that child. You will have five sections to complete to provide more extensive information with clear instructions for treatment should your student ever visit the school clinic or participate in field trips or MPC athletics. Remember to finalize answers by clicking the submit button and "complete for approval." 

 

7. Be sure to update your child's "Vital Health Record" and series of simple questions to be sure MPCS has all important information on file. Once all sections are complete, your child's medical records are secure and ready for the next school year!

 

8. If any medical information changes throughout the school year, simply access Magnus from your FACTS portal to update your child's record at any time. This is also where you may download and upload important documents from your pediatrician or sports healthcare provider. Required immunization forms (for new students, Kindergarten, new updates by age 12, and rising grade 11) and over-the-counter medicine permissions are updated here. All parents must check that their child(ren)'s medical information is updated by August 1.
