How to Upload Sports Physicals for Athletic Participation

If your child plays a sport at MPCS, they are required to obtain an athletic physical. The required form for all grade levels is the GHSA PPE form. MPC athletics offers a sports physicals day on campus each May. Parents may also obtain a completed sports physical form from your doctor or pediatrician. 

In addition, parents are required to fill out the MPCS sports interest and participation permission forms, the GHSA heat and humidity form, concussion awareness form, and sudden cardiac arrest forms. These all can be found in your FACTS portal > Magnus health section. 

Instructions on how to upload your printed physicals form and access all health forms:

1. Download the GHSA Physicals form and print all 4 pages.

2. Make sure a parent fills out pages 1-2 of the "history" form and signs it. The athlete must also sign this form. The physician must fill out pages 3-4 of the form and sign also.

3. Login to FACTS > Magnus.

4. From the Magnus "front desk" homepage, click "complete now" on your "to do" list. Start with your Physicals form.

5. There are 2 simple ways to upload the 4-page physicals form to Magnus:

• Scan the 4-page document to a PDF and upload as a single file.
• Take a photo of each page with your phone. You may find it easier to email yourself the 4 images to upload through your computer. Or you may upload each image from your phone.

6. Student-athletes are required to have all 5 forms on file. Click "fill it out" for each form. 

• GHSA Physical (good for one calendar year)
• Concussion Awareness
• Heat and Humidity
• Student Cardiac Arrest
• MPCS Athletic Participation Consent form

Remember to click the "submit" button and "complete for approval."
