MPCS Physicals Day will be May 10, 5:00-7:00 p.m. in the Adair Gymnasium, first come, first served. All MPCS student-athletes and marching band members are required to have a new physical on file in order to participate in sports or band for the upcoming school year.
The cost is $25. Each student should arrive with either cash or check (made out to MPCS), as well as the necessary forms:
- GHSA Physicals Form - This is a four page document that must be completed prior to seeing the doctor and signed by parent/guardian.
- MPCS 2022-2023 PARENT Participation Consent Form - online only
- MPCS 2022-2023 Participation Consent Form (Students age 18+) - online only
Students should wear shorts and a T-shirt. (Girls should wear a backless bathing suit or sports bra under their t-shirt.) Click here for FAQs concerning physicals day.
